I think the Scratch team should make XD: a smiley that looks like it's laughing really hard. What do you think?

There isn't really a way you could represent that as an image.
Personally, I prefer the text XD.
It conveys more meaning.

My interpretation of that is XD: is the whole emote due to the lack of caps in the A of "a smiley"

Sure, if you provide them with an image to use, they can just add that smiley with the Easysmiles modification.
