Many people in the Scratch community have asked a question like this, or "how do you make a 3-d game?", and the answer is, "In scratch you cannot make ACTUAL 3-d games because there is no Z position, though you can SIMULATE or MAKE THE ILLUSION of a 3-d game" . Though there is many types of 3-d games. Theres runescape/world of warcraft 3-d, there 1st player shooter 3-d, and theres even super smash bros 3-d, so all in all it depends what type of 3-d game you want.

No it isn't just size. An object far away will appear to move slower than an object closer to you. Plus, 3D should also allow you to rotate and move around in a 3D space.
@Caveman: search up archmage's projects, there are projects on 4-way scrolling, 2-way scrolling, etc.
@Militarydude: search up addzero's projects. He has projects using 3D projection, which places 3D coordinates onto a 2D space. Or you can try Canthiar's projects, which use slow and very complex ray-tracing, which renders a realistic image.
Hope I helped
