Hmmm, I'm not sure.  I think the third-to-last line might be throwing it off.

I thought it may have been the Funimation or Ajisai lines but maybe. When I removed the song credit the notes turned out fine, but then why in the world would that line throw it off?

It's in one of the song titles... first song's artist.

Just put a period, space, or something after the first letter of the artist name from the first song.
