One of my headphones speakers rattles inside the case 

That happens for me a lot.

When my $30 Sony Piiq headphone started doing that, they died a week later.

Yeah, that happens, especially with cheaper sets. Both of my $10 pairs have died on one side.

Well, that's what I meant by "headphones", actually.

I've never had actual (read: not earphones) headphones die on me.

I've never had a pair of headphones die on me 
Though I don't use headphones that much. I used to use a pair of clip headphones, but the clip on the left one broke while sitting in my backpack once; that's the only reason I don't use them anymore. Now I use a pair of earbuds that came with my MP3 player, and they're fine. When I'm at my dad's house, though, I always use a pair of real headphones.

A lot of my headphones break because the plugs wear out. In such a way that you have to keep squeezing them to get any good sound.
