That's a toughie. Even though I heard Ninty recommend that kids under 6 shouldn't play on 3DS because it can permanently damage their eyes, I still am strongly in dire need of the 3DS.
Although, I am using the iPod touch to type this...

The 3DS is not big. In fact:
134 mm (5.3 in) wide × 74 mm (2.9 in) deep × 21 mm (0.83 in) high (3DS)
137 mm (5.4 in) wide × 74.9 mm (2.95 in) deep × 18.9 mm (0.74 in) high (DSi)
133 mm (5.2 in) wide × 73.9 mm (2.91 in) deep × 21.87 mm (0.861 in) high (DS Lite)
It's smaller than the DSi, but bigger than the Lite.

I wouldn't worry that much about the 3D hurting eyes. It's no different than 3D movies or Tv, really. Long-term affects won't be understood until a while in the future.
You should wait until games for the 3DS come out, anyways.

I'd definitely choose the iPod touch, although I'm not much of a mobile gamer. It's got an excellent camera, beautiful display, and basically all the functionality of a self-respecting internet device. You'll still be using it long after you grow tired of a DS.

It has a camera which no normal system does, amazing graphics, at least some internet, and plenty of games with eyepopping graphics and amazing gameplay to keep you interested way after the Ipod Touch is once again outdated.

Get the 3DS for sure.
Don't worry about health effects, the 3DS just uses an updated version of those bumpy cards that appear 3D. And besides, there's that slider so you can turn the 3D off completely if you want.
The 3DS has two cameras on the outside so you can take real 3D photo and video, PSP-quality graphics, an online shop, web browser, and plans for a movie store of some kind. I'm not saying you shouldn't get the iPod touch ever, but you're just likely to get more out of the 3DS.

Yeah, jactkt is right same thing happened to me, I was saving to get an iPod Touch then I heard about an 3D DS! So go for 3DS! Also there is the fact that 3DS Game Cards Could Reach a Mighty 8GB So expect huge games. The new-style cartridges used by 3DS may look similar to those you currently slot into your DS, but they're actually bigger, on the inside at least. Manufacturer Macronix has confirmed that the game cartridges will start at 1GB capacity and run all the way up to a massive 8GB of storage, a huge increase over the DS, which currently offers 512MB as its largest available..

With the 3DS, it actually looks like the graphics are popping out (or sinking into) the screen, and you don't even need to wear special glasses to see it! That's why it's so awesome.
