Scratch

A fishing rod (never tried seafood before, but I can't starve, you know), some sort of bedding, and a boat.

If you're supposed to stay on the island, instead of a boat I'd want shelter.

A knife, to whittle.

Well, seeing as how you never defined the size of the island, if there's anything at all already on the island, and how long you would be there...(yes, I love to over analyze things) nothing.  I can honestly say I'd be fine with dying.
