I want to know how it's decided who gets to join the Scratch Team or who gets to become a moderator.

From what I can tell, every now and then, when it's decided by the Scratch Team that the forums need more moderators, they'll choose a few.

Yes, and you'll be promoted to a Member whenever a mod notices you've been good in the forums.

I think to get on the Scratch Team, you have to be linked with MIT in some way... Not sure.
As for becoming a mod... there are a couple of ways:
A) Being chosen by the Scratch Team.
B) Running in one of the mod elections (held every May, September, and January), being shortlisted for the final election candidates (OMAI, it'z The List again - ex-candidate joke xP The List makes all the candidates nervous ), then being elected by everyone else.
You have to have been active in the community for 6 months, though, and have a fairly good history (rarely being warned by the Scratch Team/mods, helping others, reporting bad stuff... and generally being nice to others
)
As for being moved to member - it will be fairly soon

You can become a community mod by been elected every few months.
Its hard but if ur a good scratcher u will

If this was the case, half the users on the forums would be running the forum, and we'd have a forum in anarchy. Happy happy!
