well, I think that standards have definately relaxed a bit, but also the system is too rigid.  games like CoD and Halo should not be rated M because they portray military violence, which in most cases isn't quite as disturbing.  however, and game with spontaneous killing (GTA, etc.) or sexual situations should be rated M.
