Scratchthumbs.db is a file Scratch scatters around my computer whenever I load a picture. Or something like that.
The reason I hate it is that I have a very neat, organized computer. I don't want a program flinging files who-knows-where.

Since you like organization how about you put all your scratch files in one folder? Genius!

@Stickman: But then it lags a lot... I have to divide all my images into seperate folders so that it doesn't lag as much...

Ah, that's scratchthumbs.db
When you see the message "Updating thumbnails" when browsing to open an icon, it creates this file. It can take a long time, as you point out, as it opens each image it finds in the folder and process it to make a miniature it displays in the browser dialog.
So, to avoid spending so much time on each browse action, it stores the result of the search in this file: if nothing changes since last visit, it can display super fast these thumbnails.
Note that Windows does the same when you ask it to display miniatures in Explorer: it creates a Thumbs.db file on each folder it has to display this way.
Except it is "smarter" and hides it: I see them because I have set up Windows to always show hidden files (I like to see what is behind the scene...).
In short: don't worry, this is made to ease your life. If you don't like lags, just leave them. If you keep your images for scratch together, at least you will find them only in the related folders.
And you can delete them safely if you prefer to wait a bit when browsing for images.

The scratchthumbs.db file is entirely meant to load thumbnails faster. It didn't yell at you or anything. What's so annoying about it?

Sadly, I tried hiding the scratchthumbs.db file (this is windows 7) but it seems when a program (a.k.a. Scatch) accesses it, it unhides it. It's very annoying.

Scratch thumbs.db is meant to be kept, without it you would have no thumbnails so you won't be able to see images, sprites ect
