There is nothing wrong with being gay, but if you haven't noticed, some people in the world use it as a really nasty insult. And I have a friend that is gay so it kinda effects me too.

I have a feeling that whoever put it there meant it in a offensive way because there really is not other answer to what it could be.

Actually, there was an advertisement for cigarettes from 80s (probably). There was a woman in a fancy dress, and her spine was made to look as, well, male organ. It was somehow related to cigarettes and the text under the photo.
