Start by looking at the examples that come with scratch.

Your question is really too general, though.  You need to think of what you want to happen in your game, then break it down into "sprites" - the characters in teh game.  What will they do, how will they interact with each other and the scenery.

Once you have those ideas in your head, you can ask for specific help on how to set it up.
