1. When a user is not allowed to access the site due to bad behavior (see #2 for more info on that)
2. Bad behavior, which can be posting inappropriate projects, swearing excessively, trolling, or being disrespectful (or just anything not nice)

If you are disrespectful or mean or spam, you get banned. Usually, a ban is 2 weeks, though the ST can change this depending on the severity of the problem or how ever many times you've been banned.

I'm not sure where you got your information - but I don't think it is right. The first block is usually half a week. The length doubles for any subsequent blocks.
Accounts are blocked when the account owner goes against the Community Guidelines. In the forums, there are additional guidelines for most sections of the forums: look for the topics at the top of the index that have been "stickied".

If they did something really bad, it can be longer. They also have to be sure to give their word that they agree to the ToU.
