Either DOOM when I was 4, or Minecraft about a year ago. Anyone who has ever played MC will know I'm serious about the last one.

There was this help screen in the Nintendo 64 Super Smash Brothers or Mario Kart Racing with some sort of creepy purple hippo and I got stuck on it when my sister was out of the room and couldn't make it go away
I was in preschool I believe, and my terror resulted in very disturbing intrusive thoughts of the hippo chasing me that I couldn't seem to make go away no matter what I did
I confided in a friend several months later and she thought it was hilarious 
(Admittedly, I also find it hilarious)

I thought the game was called like Nine Persons, Nine Doors, Nine Deaths or something?

The first time I played, my cousin dared me to play on Hard. So I did. Then it was getting dark and I barely had any supplies, so I dug a little room into a wall. I left myself a little 1x1 hole to look out through. I was really freaked out when I saw a creeper's face right in front of the hole.
Later, I decided to man up and go fight a monster with my wood sword. For some reason, I picked the tall black monster with purple eyes. So I looked at it and swung my sword. It disappeared. I was like, "Oh boy, that guy was easy!" Then the enderman teleported behind me. I tried to avoid Enderman for now on.
As I kept playing, I became a better player. Using cheats, I decided to go to the end and fight the Enderdragon. I didn't know Endermen were in the End too. 0_0
If you want something to do while Scratch is down, check out tbgs.tk! It's the new version of the (closed) Scratch TBGS, run by Scratchers! I'm going to spend most of my time there, anyway.

Okay, umm I think I meant to post that elsewhere, because the first game that scared me (and the first I ever played) was Gex: Enter the Gecko for the playstation 1.
