That happened when I tried to use a username that has already been used.

Also, whenever you use an apostrophe In your page it puts a "/" before it.

You have to stripslashes() to unescape the text.

I click on the Create button and I get a 1freehosting error message saying the page doesn't exist.

Your 404 page should say "404, man."  .
