Is it a Scratch game?
No.

Since this is not Scratch related, it looks like Making and Creating is the best fit for it.
However, I'm always suspicious of zip files that I am supposed to download and run. I've read a lot of horror stories about viruses and trojan horses. Perhaps the author would like to explain what is in the zip file and what language the game is written in and what platforms it is supposed to run on?

I can confirm it's an actual game. It appears to be made in Game Maker.

It is an actual game, no viruses (as far as I can tell...).
Some things you could do to improve it:
- The graphics are kind of sucky, especially the old lady. You could request on the requests forums for this.
- The old lady keeps going off the screen - it would be nice if it couldn't go past the gray border or something.
- The instructions should be available in-game.
