How do I make a scratch mod?

I would like to know how to and what is the easiest way.. will i need to know any programming languages?

Well, you'll need to know Squeak Smalltalk.
Download the Scratch source code and open it in Squeak.

Make a few blocks.
Test them out.
Repeat.

When you are satisfied with it, release it.

Problem is, you can't upload the project you make with it to Scratch.
