It's a thing you buy with real money that you can either wear or trade in-game for a snazzy new item. It looks like this: 
(all the images in the game look like that, but the game itself more than makes up for it)

It's an item in the MMO Kingdom of Loathing that can be used to boost attributes or traded for rare exclusives. They can only be acquired by trading, a very rare Crimbo stocking that was given to 150 random players in 2004, and by the main way, donating $10.
