Using Squeak, the source code.  It's taking programming to the next step and is rather advanced.

Re: How is it possible?

You can make new blocks by downloading the Scratch source code, then following the guides people make. Just so you know, all of Scratch is written in Squeak, so if you can learn that making your own blocks is much easier.

I'm taking a break from Scratch until 2.0 comes out. Any messages sent between then and now probably won't be read - sorry.
