You put image tags around the URL of the image. For example, if the image was at example.com/picture95.jpg, you would type [img]example.com/picture95.jpg[/img]
Do you know where the URL is for whichever picture you want to put here?
Or do you perhaps mean putting an image into the scratch program? Well if you right-click the image and select 'Save Picture', and you save it into the Scratch Folder which should be in your program files, you can import it as a sprite or costume or background directly into Scratch (but you should know that if it is larger than 360x480, it will be reduced in size and the quality might be messed up).

Right-click the image, click properties, copy the URL, paste the URL onto a post or siggy, and put [img]and[/img] around it.
