OK, I made this using the GIMP program, which you can find via a Google search. I'm sure the technique is similar in Photoshop, if you have that. I can provide screenshots if needed. My first tutorial, so it will be garbage.
Step 1: Create a new file. It can be whatever size you want. Do a right click -> select -> select all.
Step 2: Do a right click ->select -> rounded rectangle with a radius of around 15. Using the rectangle select tool, select the lower left, lower right, and upper right corners while holding down shift.
Step 3: Do a right click -> select -> border selection by one pixel. Fill this with black.
Step 4: Select an area near the top that is one pixel tall by the width of your image. Fill this with black. This creates the outline for the title bar. Select the white inside the title bar, and fill it with a gradient with a light grey at the top and a darker one at the bottom.
Step 5: Select this grey with the fuzzy select tool. Do a right click -> select -> border by one pixel again. Fill this with a white color with an opacity of 50.
Step 6: New layer. Create three square selections an equal distance apart. Right click -> save to channel. Create three squares and fill with white. Do a select none, and then a gaussian blur of about 1-3 pixels.
Step 7.Do an emboss with the azimuth at about 115. Then colorize the buttons to be the color you want. Then, go to the channels tab. Load your selection, do an invert, and delete. Now position this layer where you want it.
Step 8: New layer. Make white text in as large a font as possible while still having it fit inside the buttons, create an X, a -, and a + sign. Make sure the font is a bold one. Position these over the buttons. Do a select by color on the white and save this to a channel.
Step 9: Do a gaussian blur with a radius of about 1-2 pixels. Now do another emboss with an azimuth of 295.
Step 10: Load your last selection from the channel. Do another invert and delete.
Step 11: Add your title.
DONE!!!
