Well then:
In debug mode, in the middle box, type
Preferences enableProgrammerFacilities
Then right click and click "do it"
Then close out of the pink debug box (doesnt matter about saving the changes) and MIDDLE click on the scratch program.
You will see different symbols inside little circles around the upper edges of the scratch program. Click the O below the X on the left hand side of the scratch program. This will hide the program but not close it.
Now right click, select open, and select broswer.
Then you can edit squeak from there.
I am still learning squeak, so I can't help you from there, but the main path to add blocks are:
Scratch-Objects -> (make sure class is selected) ScriptableScratchMorph -> blockSpecs -> blockSpecs

Sorry for necro-posting,
In order to do this on a laptop with no middle click
When you get to the middle click part, in the top left corner of the scratch program you will see:
Click the cat head, and select VM preferences
Then go to Mouse Mapping and select three button mouse.
Now your right click is your middle click. The one problem is that you will have to keep checking and unchecking 3 button mouse.
Note: Do not check one button mouse, this will not revert your 3 button mouse back. You have to uncheck 3 button.
