there ist probably more than one file with the *.image extension in the folder from which you're starting Scratch (Maybe you were experimenting with the Scratch Source code or with another Scratch-Variant). In this case, just de-installing and reinstalling Scratch won't help, so I'd recommend that you either manually remove / rename the other image file, or manually delete your whole Scratch application folder and then reinstall Scratch.

The way that scratch works is that there is an executable (scratch.exe/.app), which handles the OS end of things. Then there is an image (scratch.image) which is cross-platform, and contains the actual program. If you click on the exe, and there are either no image files in its directory, or more than one, than it will ask you to browse for one. To fix this, right-click the desktop, and click 'new' then 'shortcut.' Now, when it asks for the location, rather than just one, put the executable, then the image, quoted. Like this:
"C:\Program Files\Scratch\Scratch.exe" "C:\Program Files\Scratch\Scratch.image"
