The most disturbing place that I can think of in a game is Chapter 6 Level 2 in Super Paper Mario for the Nintendo Wii.

I don't know how Rare got away with this in a childrens game. But in Banjo-tooie the boss of cloudcukooland. After you defeat it it starts shaking its head, It then slowly twistes around in circles (Thinking of a scary movie yet?) Its head then launches off like a rocket. With green blood flying everywhere. WHAT THE HECK?

Was the ESRB (American/Canadian game age rating group) Sleeping while rating this game (Theres a truck load of adult humor too)

Similar too above. The gaint fish boss (Who's name somehow got past the ESRB) BLEEDS RED BLOOD! When hit.

The rest of the game isn't scary, but BLOOD in a E Rated game? Wow. Just Wow. I wonder what all the Strict Parents would think?

All of these suggestions are great!

Giygas is the obvious choice
the gameboy camera faces are just... disturbing
ReDeads, SPECIFICALLY THE WIND WAKER ONES.
Chapter 6-2 from Super Paper Mario is plain disturbing
Zero Two is another obvious choice

May I just throw in...

The Old Chateau from Pokemon D/P/Pt (horrified me as a kid)

The Guardians from Skyward Sword (not for how they look, but just the horrifying moment you see those things out to kill you)

Tourian from metroid, specifically zero mission. (The metroids...           ...enough said)

That is all I can think of for now.

I'm in Minecraft.
Everything's peaceful.
Just walking through a nice sunlit forest, chopping down trees, when suddenly
BAM. I hear the worst sound in history... "MMMOOOOOOO"
And suddenly a COW jumps behind me, and I'm like "GAH". Most horrifying creature in Minecraft. I'll take a handful of creepers and spider jockeys (don't forget endermen as well) any day.
