When mario drowns in mario 64 its realistic and extremely scary. Im surprise it got an E rating. Oh yeah the eel and the evil panio. Scarest kid game ever.

Some scenes from Sleeping Dogs are pretty brutal, throwing people into wood chippers, impaling them onto meat-hooks, slamming their head into a table saw, embedding a cleaver into their skull, etc.

Although there's one pretty bloody scene from the Dawn of War intro where he smacks someone's head with his gun then starts cutting him in half with his chain-sword.

There's quite a few games out there with a lot of blood, some do it well and some are just badly done.

The Walking Dead (Telltale Games remake) is quite gruesome at the start; shooting a zombie in the head with a rifle - leaving a large hole, smashing a zombie's head to pieces with a hammer and stuff.

Oh boy. I somehow knew this game would come up.
It's called Custer's Revenge, it was for the Atari 2600, and is known as one of the earliest mature games. I'd post a link to the Wikipedia article, but again, it's mature and kids of all ages go on this site.

Just look it up so I can try to forget that this game existed.
